Community Partners on the Web and Social Media
We will continue to seek the involvement of our community partners as we move forward in the Community Health Improvement Plan (CHIP) process. Because of the many thriving coalitions and partnerships that have been nurtured in Platte County and the Northland, we have a huge head start in developing a comprehensive CHIP. When practical, we’ll utilize existing coalitions to focus on the identified priority health issues. Where gaps exist, we’ll form new focus groups. Regardless, we’ll seek the continued involvement and valuable input of our community partners. Together, we can take action that will lead to positive change in our community!
